**Trial Participation Release
This form is for our trial classes. We must have a signed athlete evaluation form to be accepted into any class.

**2011-2012 Registration Form
We must have a current signed registration form on file in the office before anyone can be enrolled into any of our programs. Telephone registrations are not accepted.

**Auto Pay Form
This is an optional form. You may accept the automatic payment program and understand that your credit card will be billed for tuition and other outstanding charges between the 27th-1st of each month.
